To apply, please bring copies of all the following documents. Applications must be completed and returned..
If Employed:
Pay stubs for the last 3 months for all household income
Most recent Federal Income Tax Return and W-2 Forms
Social Security Award Letter (Current Year)
Recent bank statements (checking and/or savings; bank printouts accepted)
Any pension or annuity award letters for anyone in the household
Social Security card
Copy of Drivers License or Government ID
Insurance card, if you have any type of medical or prescription coverage
AFDC award letters, including food stamp verification
Medicaid eligibility letter

How to Qualify for Services
The Broad Street Clinic is a chronic care clinic serving uninsured adults who are medically and financially in need. We provide care for patients managing the following chronic conditions:
Diabetes
High Blood Pressure
Heart Disease
Lung Disease
Thyroid Disease
Please note: If you are currently enrolled in Medicaid, you are not eligible for services at the Broad Street Clinic.
Eligibility & Application Process
Eligibility is based on:
Medical need (diagnosis of a qualifying chronic condition)
Financial need (determined through income documentation)
All patients must complete an eligibility screening with their Patient Advocate. Eligibility must be renewed annually.
